O-11-214 Create SUPERVISOR FOLLOW UP OF CLINICALLY SIGNIFICANT EVENT Note
🎯 Purpose:
Entered by a Clinical Supervisor after being notified of a clinically significant event that requires follow-up or delegation. The Clinical Supervisor will keep the coordination note in workflow until the issue has been resolved.
When a clinically significant event is escalated to the Clinical Supervisor, the Clinical Supervisor shall promptly review the issue and take appropriate action to ensure client safety and compliance with agency requirements.

- Review of the Issue:
- Review the clinician's assessment, supporting documentation, and actions already taken.
- Assessment of Immediate Risk:
- Determine the actual or potential impact on the client's health, safety, treatment plan, or medication regimen.
- Select SUPERVISOR FOLLOW UP OF CLINICALLY SIGNIFICANT EVENT note:

- Provider Notification and Follow Up:
- Verify the ordering provider has been notified of the clinically significant event.
- If provider notification has not occurred, or if no response has been received, initiate additional provider contact and document all attempts
- Clearly communicate the identified issue, clinical findings, and need for provider direction.
