O-11-214 Create SUPERVISOR FOLLOW UP OF CLINICALLY SIGNIFICANT EVENT Note

🎯 Purpose:

Entered by a Clinical Supervisor after being notified of a clinically significant event that requires follow-up or delegation. The Clinical Supervisor will keep the coordination note in workflow until the issue has been resolved.


When a clinically significant event is escalated to the Clinical Supervisor, the Clinical Supervisor shall promptly review the issue and take appropriate action to ensure client safety and compliance with agency requirements.

Refer to O-11-200 Create/Review Coordination Notes in Back Office for detailed steps on how to create a coordination note in PointCare.

  1. Review of the Issue:
    1. Review the clinician's assessment, supporting documentation, and actions already taken.
  2. Assessment of Immediate Risk:
    1. Determine the actual or potential impact on the client's health, safety, treatment plan, or medication regimen.
  3. Select SUPERVISOR FOLLOW UP OF CLINICALLY SIGNIFICANT EVENT note:

  1. Provider Notification and Follow Up:
    1. Verify the ordering provider has been notified of the clinically significant event.
    2. If provider notification has not occurred, or if no response has been received, initiate additional provider contact and document all attempts
    3. Clearly communicate the identified issue, clinical findings, and need for provider direction.
