O-13-300 Refer to Mobile Wound Clinic

Responsible Position: Clinical Supervisor

🎯Purpose:

To describe the process for referring a client to a Mobile Wound Clinic for consultation and assistance with a wound assessment to determine the best possible product and an acceptable visit frequency to allow maximum wound healing and oversight.


Mobile Wound Clinics bill for provider services and handle their own billing. They offer wound consultations by a physician and/or and advance practice nurse. Adara should coordinate the home visits with the mobile wound clinic so the Adara home visit and the Mobile wound clinic visit do NOT occur on the same day.


Field staff will evaluate the need for a mobile wound referral and escalate to a Clinical Supervisor.

🔄Process:

  1. Ensure the wound is appropriate to refer to the Mobile Wound Clinic, including the ability for the Mobile Wound Clinic to bill the client’s payer.
    1. Refer to O-13-301 Mobile Wound Clinic Resources for available Mobile Wound Clinics and payers accepted.
    2. Refer to O-13-302 Indicators for Mobile Wound Consult.
  2. Ensure there is an order in our EMR from the home health ordering provider for a mobile wound consult.
  3. Contact the Mobile Wound Clinic and ask them if they will accept our referral (refer to O-13-301 Mobile Wound Clinic Resources) and enter this information within a Clinical Note to tack the progress of the referral.
  4. Send the following information to the Mobile Wound Clinic via fax:
    1. The client face sheet
    2. Any H&P that describes a recent vascular consult, if available.
    3. The most recent visit note with wound description/order and assessment
    4. The order obtained for the wound consult which will include the name of the primary care provider.
    5. Name of home health Case Manager and Clinical Supervisor, phone and fax number of the branch office
  5. Once confirmation is received the referral is accepted, communicate Adara’s home health visit schedule to the mobile wound clinic. Adara will not provide a visit on the same day as the Mobile Wound Clinic, unless it is for training purposes or for a reason unrelated to the wound care.
  6. Ask the mobile wound clinic for a visit schedule to be sent to the office by fax when it is known. Inform the mobile wound clinic that changes to the mobile wound clinic provider’s visit schedule be communicated to the case manager and/or office.
  7. Ensure the mobile wound clinic is listed as a provider in HCHB (add a Physician if necessary).

  1. Enter the Mobile Wound Clinic as a Client Contact:

  1. Enter a Point Care Visit Alert which should include the Mobile Wound Clinic that is involved, the name and contact information of the provider is that is following the wound and the day of the week the provider is planning to visit the client.
  2. Update the Case Manager on the status of the referral.
    1. Provide the name and contact information of the Mobile Wound Clinic.
    2. Remind them that wound care supplies should be left in the home for the Mobile Wound Clinic Consult

The Mobile Wound Clinic will:

  1. Communicate and coordinate their home care visit with the office and/or Case Manager.
  2. Ensure Adara has any new wound care order, if applicable.
  3. Ensure Adara is made aware if they are no longer following the client for care.
  4. Provide documentation following Advanced Wound Care Modalities for treatments such as tissue grafting with wound measurements to incorporate into our records, when indicated.
