O-13-302 Indicators for a Mobile Wound Consult
🎯Purpose:
Identifies common identifiers that may alert Clinical Supervisors to an appropriate referral for a Mobile Wound Consult.
NOTE: Referrals for surgical wounds within 90 days of the operation, or those being actively followed by another provider, are generally not appropriate for a referral unless specifically transferred to a wound-specific provider.
🔄Process:
Evaluate client record for the following indicators:
Assessment Findings: Clinician may initiate the request based on the following clinical assessment findings:
1. Stalled wounds (wounds that have not improved in 2 weeks)
2. Any wound that has worsened under our care
3. Any wound requiring 3 or more home health visits per week
4. Any complex wound or a client with 3 or more wounds
5. Any wound larger than 3x3cm
6. Any client with mobility issues or concerns that complicate wound care
7. Any wound that is deeper than epidermal level
8. Any client with mobility issues or concerns that complicate wound care
WAT Scores:
1. Significant deviations in WAT score
- Process Review Wound Score Deviation workflow task
- Review Significant Wound Score Change Report
Clinical Record Review:
1. Review Infection QI Reports for wounds with two or more infections within a 60 day certification period
