🎯 Purpose
Facilities in HCHB include organizations and locations to be used as referral sources (i.e. Clinics, County Social Worker/Public Health), service locations (i.e. Assisted Living Facilities), and Inpatient Events. They also include Pharmacies, DME Providers, Infusion companies and Laboratories. This process details the steps required to add a new facility in HCHB.
🔄 Process
Go to the Orb
Select Clinical Manager Maintain Facilities
- Search for the Facility first
Duplicate facilities result in inaccurate referral tracking
If Facility is not listed, select "Add +"
- Enter all required fields designated with red asterisk:
Facility Name:
Facility Type:
Address (Must Be Validated):
Service Line: Home Health:
- Enter the following when applicable:
HH Admission Source: Required for facility to be selected as an Inpatient Event:
- 61-Acute Care Hospital: Most hospital facilities
- 62-Inpatient Psychiatric Facility
- 62-Inpatient Rehab Facility: May be a freestanding rehab facility OR a rehabilitation bed in a rehabilitation distinct part unit of a general acute care hospital
- 62-Long Term Care Hospital: Also known as Long Term Acute Care Facilities (Example in MN is Fairview Bethesda Hospital)
- 62-Skilled Nursing Facility: Includes SNF, TCU, Nursing Home and Long Term Nursing Facility
Referring Facility: Required for facility to be selected on a referral
Inpatient Facility: Required for a facility to be selected as a Service Location (i.e. Assisted Living Facility) OR Inpatient Event
Select Save & Close
If NPI is missing, user will receive a prompt to correct. Select "No" if facility NPI is not known
Every facility must be associated to at least one branch. Select "Yes" to associate facility with branch:
Select appropriate branch/branches from drop down, then select Save & Close
- Facility is now ready to be selected.
