O-11-203 Initiate a Request to Add New Physician

🎯 Purpose

When a user identifies that a new physician or non-physician practitioner (NPP) needs to be entered into Home Care Home Base (HCHB), the user enters a “REQUEST TO ADD NEW PHYSICIAN” coordination note. This request will route to the Medical Records Specialist for review and entry into HCHB. The process below identifies the steps needed to initiate a request for a new provider entry in HCHB. 

🔄 Process

1. In HCHB, select Clinical Input. 

   

2. Search for and select the client associated to the new physician/NPP request.

3. Access the client’s Coordination Notes.

   

4. Select “Add”.

   

5. From the dropdown, select the note type “REQUEST TO ADD NEW PHYSICIAN”.

   

6. Complete the template with as much information as possible.

   

7. Select “Save”.

   

8. Medical Records Specialist will email note creator once provider has been entered into HCHB.
