🎯 Purpose

This task generates to the Administrative Specialist to facilitate the printing and sending of the Discharge Summary upon discharge from agency. This task is intended to meet the Home Health COP G1022 (6)(i) “A completed discharge summary that is sent to the primary care practitioner or other health care professional who will be responsible for providing care and services to the patient after discharge from the HHA (if any) within 5 business days of the patient’s discharge.”

1. Select Process Episode Summary to Physician Task: 

2. Three options are available to print and review. Select only the third option: “Review Discharge-Transfer Summary Report”:

3. Select Print Icon and print to PDF to generate file to send to provider: 

4. Physician Name and Fax is listed at the top of the report for reference: 

5. Once fax has been sent successfully, select “Stage Completed”:
